Driver License Forms and Applications

  • Change of Address by Mail - DMV 22: Complete this form and mail it to the DMV to change the address on your driver license, CDL, identification card, instruction/learner's permit
  • Nevada Driver's Handbook - Questions on the Nevada permit test and written driver exam come from the Nevada Driver Handbook. Reading through the handbook will help you get a better score on your written exam.
  • Application for Nevada Driver License by Mail - DMV 204: Nevada residents who are temporarily residing outside Nevada and meet all other Department requirements may use this form to apply for a driver’s license renewal or duplicate by mail.
  • Application for Driving Privileges or ID Card - DMV 002: Nevada residents who cannot meet the proof of identity requirements for a driver's license or ID card may be able to obtain a Driver Authorization Card (DAC).   • Beginning Driver Experience Log DLD 130 - Required to complete Nevada's Parents Supervised Driving Program. The Beginning Driver Experience Log should be updated after each driving session to prove you have completed 50 hours of behind-the-wheel practice by the time of your skills test

Vehicle Registration Forms

  • Application for Vehicle Registration - VP 222: Necessary to apply for a new vehicle registration. Evidence of insurance must be presented at the time of application.
  • Personalized License Plate Application – SP66: The DMV reserves can refuse any combination of letters and/or numbers that are deemed to be derogatory, vulgar, or in conflicting in other ways
  • Vehicle Inspection Certificate - VP 15: This form must be completed by an Authorized Nevada DMV Representative or a Peace Officer in order to verify a Vehicle Identification Number (VIN)
